DESCRIPTION
Gentam is a complex antibiotic for animals (International nonproprietary name: amoxicillin + gentamicin).
STRUCTURE
1 ml contains 150 mg of amoxicillin trihydrate and 40 mg of gentamicin sulfate, as well as auxiliary components: butyl hydroxytoluene, magnesium stearate, benzyl alcohol and vegetable oil.
PHARMACOLOGICAL PROPERTIES
Amoxicillin trihydrate and gentamicin sulfate exhibit synergistic effects against a wide range of microorganisms that cause infectious diseases in animals: Staphylococcus spp., Streptococcus spp., Actinomyces spp., Meningococcus spp., Escherichia spp., Shigella spp., Salmonella spp., Pasteurella spp., Listeria monocytogenes, Clostridium spp., Haemophilus spp., Bacillus spp., Actinobacillus spp., Bordetella spp., Fusobacterium spp., Moraxella spp., Corynebacterium spp., Campylobacter spp., Erysipelothrix rhusiopathiae, Proteus mirabilis, and also on penicillinase-forming bacteria Klebsiella spp, Enterobacter spp., Pseudomonas spp, Acinetobacter spp. Amoxicillin trihydrate inhibits the synthesis of the bacterial cell wall, and gentamicin sulfate (an aminoglycoside antibiotic), penetrating the bacterial cell membrane, irreversibly binds to the 30S subunit of bacterial ribosomes, thereby disrupting the process of protein synthesis. After the injection of Gentam, the maximum concentration of gentamicin sulfate in the blood serum is noted after 30 to 40 minutes, amoxicillin trihydrate-after 1 hour and is kept at the therapeutic level for about a day. The active substances of the drug are excreted from the body mainly in unchanged form with urine and partially with bile. Gentam, according to the degree of exposure to the body of warm-blooded animals, is a moderately dangerous substance.
INDICATIONS
It is prescribed for therapeutic purposes to horses, cattle, sheep, pigs, dogs and cats for infectious diseases of the gastrointestinal tract, respiratory tract, genitourinary system, joints, soft tissues and skin, including necrobacteriosis, umbilical sepsis, atrophic rhinitis of pigs, mastitis-metritis-agalactia syndrome and other primary and secondary infections of bacterial etiology, the pathogens of which are sensitive to penicillins and aminoglycosides.
DOSAGE AND METHOD OF ADMINISTRATION
Gentam is administered to animals subcutaneously or intramuscularly at a dose of 1 ml per 10 kg of body weight, which corresponds to 15 mg of amoxicillin trihydrate and 4 mg of gentamicin sulfate per 1 kg of animal weight. The duration of the course of treatment, depending on the severity of the disease, is 2 to 5 days, which is determined individually in a particular case by a veterinarian.
The maximum volume of the drug for administration at one point should not exceed 20 ml for cattle and horses, 10 ml for pigs, sheep and calves, 5 ml for piglets, 2.5 ml for dogs and cats.
Before use, the bottle with the suspension should be shaken thoroughly for 1 to 2 minutes. The injection site after the introduction of Gentam is recommended to be lightly massaged. You should avoid skipping the next dose of the drug, as this can lead to a decrease in therapeutic effectiveness and the development of superinfection. In case of missing one dose, the use of the drug is resumed as soon as possible according to the above scheme.
SIDE EFFECTS
In rare cases, with increased individual sensitivity and violation of the order of taking and dosing of the drug, allergic reactions, refusal to feed, vomiting, impaired coordination of movements, pronounced swelling at the site of administration of Gentam, diarrhea, with prolonged use in high doses, neurotoxic reactions and thrombocytopenia are noted. In these cases, intravenous anticholinesterase drugs, calcium preparations, symptomatic and desensitizing therapy, as well as detoxification of the body are administered. At the injection site, minor edema may develop, which resolves spontaneously within 1 to 2 days and does not require treatment.
CONTRAINDICATIONS
Increased individual sensitivity to aminoglycoside and penicillin antibiotics. It is forbidden to administer the drug intravenously. Do not use in animals with severe renal insufficiency with azotemia and uremic syndrome, with a history of auditory nerve neuritis and myasthenia gravis. It is forbidden to use the drug to females during pregnancy. Gentam should not be mixed with other drugs in the same syringe, as well as used simultaneously with antimicrobial agents of bacteriostatic action (inhibiting the growth of microorganisms, but not killing them) - macrolides, chloramphenicol, lincosamides, tetracyclines, sulfonamides, cephalosporins and fluoroquinolones. To reduce the likelihood of increased toxic properties, Gentam should not be prescribed with chemotherapeutic drugs that have ototoxicity and nephrotoxicity. The drug is not compatible with muscle relaxants.
SPECIAL INSTRUCTIONS
Slaughter of animals for meat is allowed no earlier than 30 days after the last application of Gentam. The meat of animals forcibly killed before the expiration of the specified period can be used as food for fur-bearing animals. Milk of dairy animals is allowed to be used for food purposes, not earlier than 96 hours after the last administration of the drug. Milk obtained earlier than the specified period can be used after boiling in animal feed.
SHELF LIFE AND STORAGE
With caution. In a dry place, protected from light, out of the reach of children and animals. Separately from food and feed in the manufacturer's packaging at a temperature of 5 to 25 °C. Shelf life — 2 years. Opened vials with the drug can be stored for no more than 28 days.
