Inside.
Initial and maintenance doses of Glimepiride is determined individually based on the results of regular monitoring of the concentration of glucose in the blood.
Initial dose and dose selection
At the beginning of the treatment prescribed 1 mg of Glimepiride once a day. In achieving optimal therapeutic effect it is recommended to take this dose as a maintenance. In the absence of glycemic control daily dose should be gradually increased under the regular control of the concentration of glucose in the blood (at intervals of 1-2 weeks) to 2, 3 or 4 mg/day. Doses of more than 4 mg/day are effective only in exceptional cases. Maximum recommended daily dose — 6 mg.
Use in combination with Metformin
In the absence of glycemic control in patients taking Metformin, may be initiated concomitant therapy with Glimepiride. While maintaining the Metformin dose at the same level of treatment with Glimepiride starts with the minimum dose and then the dose is gradually increased depending on the desired level of glycemic control up to a maximum daily dose. Combination therapy should be under close medical supervision.
Use in combination with insulin
In cases when you fail to achieve glycemic control by taking the maximum dose of Glimepiride monotherapy or in combination with maximum doses of Metformin, possible combination of Glimepiride insulin. In this case, the last assigned patient dose Glimepiride remains unchanged. While insulin treatment starts with the minimum dose, with subsequent gradual increase of the dose under the control of the concentration of glucose in the blood. Combined treatment requires mandatory medical monitoring.
Time and frequency rate of reception of daily dose determined by a physician taking into account the lifestyle of the patient. As a rule, rather the purpose of the daily dose at once directly before or during a hearty Breakfast or the first main meal. Tablets of Glimepiride taking whole without chewing, with sufficient liquid (about 0.5 Cup). It is very important not to skip meals after taking Glimepiride.
The duration of treatment
As a rule, treatment with Glimepiride is long.
The transfer of the patient from another oral hypoglycemic drug glimepiride
When transferring patients from another oral hypoglycemic drug Glimepiride initial daily the last dose should be 1 mg (even if the patient is transferred to Glimepiride maximum dose of another oral hypoglycemic drug). Any increase in the dose of Glimepiride should be carried out gradually, in accordance with the recommendations above. It is necessary to take into account the effectiveness, the dose and duration used hypoglycemic drugs. In some cases, especially when taking hypoglycemic drugs with a higher T1/2 (e.g. chlorpropamide), you may need a temporary (for few days) cessation of treatment to avoid additive effects, increasing the risk of hypoglycemia.
The transfer of the patient from insulin to Glimepiride
In exceptional cases, during insulin therapy in patients with diabetes mellitus type 2, compensation of the disease and with preserved secretory function of the &beta-cells of the pancreas, it is possible to replace insulin Glimepiride. The translation should be under close medical supervision. The patient on Glimepiride start with a minimum dose of 1 mg.
