Composition

1 tablet contains active substance: valsartan - 80,0 mg and hydrochlorothiazide - 12.5 mg, 

1 tablet contains active substance: valsartan - 160,0 mg and hydrochlorothiazide - 12.5 mg,

1 tablet contains active substance: valsartan - 160,0 mg and hydrochlorothiazide 25.0 per mg.

Pharmacological action

Duopress - antihypertensive combination means, which comprises an antagonist of angiotensin II receptor and thiazide diuretic. 

Valsartan. Valsartan is a selective angiotensin II receptor antagonist for oral administration, non-protein nature. It has a selective antagonistic action on the receptor subtype AT1. The consequence of the blockade AT1 - receptors is increased plasma concentration of angiotensin II, which can stimulate unlocked AT2-receptor subtype, which balances the effects associated with the stimulation of AT1-receptors. Valsartan has no agonist activity against AT1 - receptors. His affinity for the receptor subtype AT1, approximately 20,000 times more, than for the AT2 receptor subtype. Valsartan does not inhibit angiotensin converting enzyme (ACE), also known under the name of kininase II, which converts angiotensin I to angiotensin II and destroys bradykinin. In connection with the lack of impact on enzyme effects bradykinin and substance P not potenziruetsa, so when taking antagonists of receptors of angiotensin II is unlikely the development of a dry cough. Valsartan does not enter into interaction and does not block other hormone receptors or ion channels involved in the regulation of the function of the cardiovascular system. In the treatment of hypertension valsartan reduces blood pressure (BP) without affecting heart rate (HR). After intake of a single dose of valsartan the antihypertensive effect develops within 2 hours, and maximum reduction in blood pressure is achieved within 4-6 hours. The antihypertensive effect of valsartan persists for 24 hours. When re-appointments valsartan maximum decrease in blood pressure, regardless of dose, is achieved after 2-4 weeks and is maintained at that level during long-term therapy. Combination with hydrochlorothiazide allows to achieve significant additional blood pressure reduction. The sudden discontinuation valsartan is not accompanied by a syndrome "cancel" (sharp rise in blood pressure or other undesirable clinical consequences). 

Hydrochlorothiazide. Thiazide diuretic, the diuretic effect of which is associated with impaired reabsorption of sodium ions, chlorine, potassium, magnesium, water in the distal nephron, delays the excretion of calcium, uric acid. Has hypotensive action, which is caused by expansion of the arterioles. Almost no effect on normal blood pressure parameters. Diuretic effect develops in 1-2 hours after receiving the drug inside, reaching a maximum after 4 hours and lasts 6-12 hours. Antihypertensive activity occurs within 3-4 day, but to achieve optimal therapeutic effect may be required 3-4 of the week.

Indications for use

Arterial hypertension (in patients, which shows the combined therapy).

Contraindications

Hypersensitivity to valsartan, hydrochlorothiazide to other sulfonamide derivatives or to other components of drug, pregnancy, pregnancy planning, lactation, severe violations of the liver (more 9 points on a scale child-Pugh), anuria, severe renal dysfunction (creatinine clearance (CC) less than 30 ml/min (0.5 ml/sec)), age to 18 years (the efficacy and safety of valsartan in children has not been established). Concurrent use with aliskiren in patients with diabetes type 2 diabetes.

Method of application and doses

Before the start of drug therapy Duopress need to correct water and electrolyte disorders. Inside, regardless of meal times, the multiplicity of reception - 1 time per day. The tablet should be swallowed whole, liquid squeezed. Depending on the clinical situation recommended daily dose is 1 tablet of the drug Duopress containing valsartan hydrochlorothiazide at a dose of 80 mg + 12,5 mg, 160 mg + 12.5 mg and the maximum 160 mg + 25 mg. the Maximal antihypertensive effect of the drug Duopress develops within 2-4 weeks of therapy. Patients with impaired renal function (QC over 30 ml/min (0.5 ml / sec)) is not required to change doses of the drug. Duopress is not recommended in patients with severe violations of the liver (more 9 points on a scale child-Pugh). In patients with mild or moderate impaired hepatic function without the phenomena related cholestasis the dose of valsartan should not exceed 80 mg.
