Pharmacokinetics

Side effect
Anemia, eosinophilia, leukopenia, neutropenia, positive Coombs reaction, increased activity of liver enzymes or bilirubin in the blood serum; candidiasis, pseudomembranous colitis, Allergy (skin irritation, rash, acne, urticaria, fever); thrombophlebitis (when I/V injection); pain at the injection site (at I/m introduction).
Interaction
The probenecid prolongs the output and causes an increase in Cmax. In combination with aminoglycosides there is an additive effect.
Method of application and doses
V/m, V/V.
Adults-750 mg 3 times a day in / m or/in, in severe infections — 1.5 g 3 times a day in/in (if necessary, you can enter every 6 hours, maximum — up to 3-6 g/day). Some infections effectively assign Zinacef at a dose of 750 mg or 1.5 g 2 times per day (in/m or/in), followed by ingestion.
Children-30-100 mg/kg/day in 3-4 doses (for most infections the optimal dose — 60 mg/kg / day). Neonates 30 to 100 mg/kg/day in 2-3 reception.
Gonorrhea: 1.5 g once/m (2 injections of 750 mg in both buttocks).
Meningitis: for the treatment of bacterial meningitis caused by sensitive microorganisms, Zinacef can be used as a monotherapy. Adults-3 g / in every 8 h; children-150-250 mg / kg / day in / in 3-4 admission; newborn-100 mg/kg / day in / in.
Prevention of postoperative complications in operations on the abdominal organs, pelvic and orthopaedic Zinacef at a dose of 1.5 g is injected into/in during the induction of anesthesia. Sequential therapy
Pneumonia: 1.5 g Zinacef 2-3 times per day (in/in or/m) for 48-72 h, followed by administration of Zinnat in the dose of 500 mg 2 times a day for 7-10 days.
Exacerbation of chronic bronchitis: 750 mg Zinacef 2-3 times per day (in/in or/m) for 48-72 h, followed by the appointment Zinnat 500 mg 2 times a day for 5-10 days.
The duration of each period (parenteral therapy and oral) is determined by the severity of the infection and the General condition of the patient.
Renal failure: cefuroxime is excreted by the kidneys, with renal failure, it is recommended to reduce the dose of Zinacef (see table.). However, there is no need to reduce the standard dose (750 mg-1.5 g 3 times a day) at a value of creatinine Cl >20 ml/min.

Patients who are on hemodialysis, at the end of each session of hemodialysis, it is necessary to introduce an additional dose of Zinacef, equal to 750 mg.In addition to parenteral administration, cefuroxime may be administered together with solutions for peritoneal dialysis (250 mg for every 2 liters of dialysis fluid). Patients in the intensive care unit on continuous hemodialysis using arteriovenous shunt or high-speed hemofiltration are recommended dose 750 mg 2 times a day. If hemofiltration is used at a low rate, then doses are used as in renal failure.

Solution for I / m administration
Add 1 ml of water for injection to 250 mg Zinacef or 3 ml of water for injection to 750 mg Zinacef. Shake gently until a suspension is formed.

Dissolve:
- 250 mg Zinacef in 2 ml or more of water for injection,
- 750 mg Zinacef in 6 ml or more of water for injection,
- 1.5 g of Zinacef in 15 ml or more of water for injection.
Mortar for on/in infusions
For short-term in/in infusions (up to 30 minutes) 1,5 g of the drug is dissolved in 50 ml of water for injection. These solutions can be injected directly into the vein or into the tube of the infusion system.
Precautionary measures
Do not mix in the same syringe with aminoglycoside antibiotics. With caution used in allergic to penicillin in the anamnesis, as well as in combination with high doses Of zinacef with furosemide. In patients with impaired renal function the dose should be adjusted based on Cl creatinine.
