Pharmacological action:
Blocks sodium channels and is a beta-blocker (refers to IC and class II), reduces the maximum depolarization rate of phase 0 of the action potential and its amplitude.
Description of pharmacological action:
Slows conduction in Atria, AV node, and system gisa — purkine. Extends the refractory period in additional conductive paths, blocking the conductivity in any direction. Renders weakly expressed beta-adrenoblokirtee action.
Clinical pharmacology:
Highly effective for the prevention and treatment of all forms of ventricular arrhythmias, including arrhythmias, resistant to other drugs with supraventricular arrhythmias, the effectiveness is somewhat lower.
Indications:
Symptomatic, requiring treatment of supraventricular tachycardia, including WPW-syndrome, or paroxysmal atrial flicker. Severe and life-threatening symptomatic ventricular tachycardia.
Contraindications:
Severe heart failure, cardiogenic shock (except arrhythmogenic), severe bradycardia, AV and intraventricular conduction disorders, sinus node weakness syndrome, electrolyte disorders, obstructive pulmonary disease, hypotension, myasthenia gravis.
Use during pregnancy and breast-feeding:
During pregnancy, especially in the first trimester, and during lactation take only according to the indications.
Side effect:
Perhaps proaritmogennoe action, increased heart failure, orthostatic hypotension, dyspeptic disorders, headache, visual and taste disorders, fatigue.
Drug interaction:
The effect is enhanced by beta-blockers, tricyclic antidepressants, cimetidine increases the level of rhythm in blood plasma. Can increase blood concentrations of propranolol, metoprolol, cyclosporine and digoxin, potentiate the action of anticoagulants.
Dosage and administration:
In/in, inside, after eating, without chewing, drinking a small amount of liquid. The dose and scheme of treatment are selected individually. During the selection of the dose and for maintenance therapy daily dose is 450-600 mg maximum daily dose-900 mg in 3 doses. Intravenous injection should be administered slowly over 3-5 min. Single dose of 1 mg/kg of body weight (with an average weight of 70 kg corresponding to 20 ml). If necessary, you can increase the single dose to 2 mg/kg (with a body weight of 70 kg, this corresponds to 40 ml). With long-term infusion is sufficient daily dose of 560 mg (which corresponds to 160 ml).
Precautionary measures:
When therapy with the drug may change the thresholds of stimulation and sensitivity of pacemakers.
