The composition and form of issue: 

Tablets, film-coated, 1 tablet contains:

caffeine 100 mg

ergotamine tartrate 1 mg

in packages contour cell or test tubes-10 PCs. in cardboard pack 1 or 2 packages or 1 vial.

Description pharmacological action:

Effect Coffetamin associated with a vasoconstrictor effect of ergotamine and improved under the influence of caffeine functions in the brain.

Ergotamine causes vasoconstrictor action.

Caffeine speeds up the absorption of ergotamine and increases the effect of ergot alkaloids in the treatment of migraine. Caffeine also causes stimulation of the CNS, mainly cerebral cortex, the respiratory and vasomotor centers. Enhances mental and physical performance, decreases drowsiness, the feeling of fatigue. Caffeine has a pronounced cardiotonic action: increases force and frequency of cardiac contractions, increases blood pressure in hypotension.

1 Cafergot tablet contains: 100 mg; ergotamine tartrate 1 mg.

  • vazopressina form of migraine
  • hypotension
  • in lowering intracranial pressure in vascular, traumatic and infectious lesions of the CNS.

Method of application and dose: 

Cafergot is prescribed inside of 1-2 tablets during an attack of headache 2 times a day, then 1 tablet 2-3 times a day for several days (up to 1 month). The maximum single dose - 2 tablets daily - 4 tablets. 

It is not recommended to use Cafergot long (to avoid the phenomenon of ergotism): after 7 days of application in cases requiring longer treatment, make a break (3-4 days).

Overdose: 

Symptoms: anxiety, nervousness, irritability, insomnia, facial flushing, increased urine output, nausea, vomiting, muscle twitching, tachycardia or arrhythmia, psychomotor agitation, tremor, hypertension, confusion, hyperreflexia, numbness in the fingers of the hands and feet, seizures. 

Treatment: removal Coffetamin gastric lavage, the appointment activated carbon, if necessary, - symptomatic therapy of beta-blockers (propranolol) warn cardiotoxicity and normalize the blood pressure.

Contraindications:

  • hypersensitivity to Coffetamin
  • decompensated cardiovascular disease, severe hypertension or atherosclerosis
  • pregnancy and lactation. 

Cafergot should be used with caution (relative contraindications):

  • insomnia, sleep disorders
  • anxiety
  • anxiety disorders (agoraphobia, panic disorder)
  • organic diseases of the cardiovascular system (myocardial infarction, angina, atherosclerosis, paroxysmal tachycardia, ventricular extrasystoles, arterial hypertension, occlusive peripheral vascular disease)
  • hepatic and/or renal failure
  • glaucoma (increased intraocular pressure)
  • old age.

Use in pregnancy and lactation: 

Cafergot is contraindicated during pregnancy and lactation.

Side effects: 

From the nervous system: agitation, anxiety, tremor, restlessness, headache, dizziness, convulsions, paresthesia in the extremities, increased reflexes, tachypnea, insomnia with the sudden cancellation Coffetamin - increased inhibition of the CNS, fatigue, drowsiness. 

From the side of cardiovascular system: palpitations, tachycardia, cardialgia, hypertension (high blood pressure). 

From the digestive system: nausea, vomiting, diarrhea. 

Allergic reactions: swelling, itching. 

Other: long-term use Coffetamin - addictive, drug dependency, weakness in the legs, myalgia, Leriche's syndrome (cyanosis, lack of pulse on lower extremities, pain, disturbance of sensitivity on the distal type).

Special instructions and precautions: 

Before applying Coffetamin you should consult a doctor. 

Care should be taken to avoid overdose Coffetamin. 

Not recommended prolonged use Coffetamin.

Drug interactions: 

Cafergot enhances the pharmacological effects of other drugs containing ergot alkaloids, and caffeine. 

In a joint application Coffetamin and barbiturates, primidona, anticonvulsants (hydantoin derivatives, phenytoin may increase metabolism of and increase in clearance of caffeine. 

In a joint application Coffetamin and cimetidine, oral contraceptives, disulfiram, ciprofloxacin, norfloxacin may decrease the metabolism of caffeine in the liver, slowing its excretion and increase concentration in the blood. 

In a joint application Coffetamin and means that cause CNS stimulation, possible excessive stimulation of the CNS. 

Caffeine is an antagonist of adenosine. 

Meksiletin reduces the elimination of caffeine by up to 50% of the nicotine increases the clearance rate of caffeine. 

Large doses of caffeine can cause the development of dangerous cardiac arrhythmias or marked increase in blood pressure in patients receiving MAO inhibitors (furazolidone, procarbazine, selegiline). 

Caffeine reduces the absorption of calcium (calcium gluconate, calcium carbonate, calcium and vitamin D) in the gastrointestinal tract. 

Cafergot reduces the effect of narcotic analgesics and hypnotics drugs increase the excretion of lithium products (lithium carbonate) urine, accelerates absorption and enhances the action of cardiac glycosides (digoxin, clanid), increases their toxicity. 

The combined use Coffetamin with beta-blockers (atenolol, metoprolol, nebivolol) may result in mutual inhibition of therapeutic effects with adrenergic bronchodilators (clenbuterol, salbutamol, fenoterol) - for additional stimulation of the CNS and additive toxic effects. 

Caffeine may decrease the clearance of theophylline and aminophylline, increasing the possibility of additive pharmacodynamic and toxic effects. 

Vasoconstrictor effect Coffetamin enhance alpha-agonists, beta-blockers, serotonin agonists (e.g. sumatriptan) and nicotine. 

Macrolides (erythromycin, spiramycin, josamycin, midecamycin, clarithromycin, azithromycin, roxithromycin) increase toxicity of ergotamine.
