Pharmachologic effect: antidepressive.
Dosing and Administration
Inside, 1 time per day regardless of the meal.
Depression of moderate and severe
Typically prescribed 10 mg once a day. Depending on the individual patient response, dosage may be increased to a maximum of - 20 mg / day. Antidepressant effect usually develops within 2-4 weeks after starting treatment. After the disappearance of the symptoms of depression should continue therapy for fixing the resulting effect for at least 6 months.
Panic disorder with or without agoraphobia
During the 1st week of treatment recommended dose of 5 mg / day, which is then increased to 10 mg / day. Depending on the individual patient response, dosage may be increased to a maximum of - 20 mg / day. The maximum therapeutic effect is reached after about 3 months after initiation of treatment. The therapy lasts several months.
Social anxiety disorder (social phobia)
Typically prescribed 10 mg 1 time per day. The weakening of symptoms usually occurs within 2-4 weeks after starting treatment. Depending on the individual patient response, dosage may subsequently be reduced to 5 mg / day and increased to a maximum - 20 mg / day. Since social anxiety disorder is a disease with a chronic course, the minimum recommended duration of the therapeutic course is 12 weeks. In order to prevent recurrence of the disease re-treatment may be administered for 6 months or longer, depending on the individual patient response.
Before the appointment of the drug should be distinguished from the ordinary social phobia shyness or timidity.
Generalized anxiety disorder
The recommended initial dose - 10 mg 1 time per day. Depending on the individual patient response, dosage may be increased to a maximum of - 20 mg / day. Allowed long-term use of the drug (6 months or longer) at 20 mg / day.
Special patient groups
Elderly (over 65 years). It is recommended to use half the usual recommended dose - 5 mg / day. The maximum dose - 10 mg / day.
Reduced kidney function. With an average degree of severity of renal failure dose adjustment is required. Patients with severe renal insufficiency (Cl creatinine less than 30 mL / min), the drug is prescribed with caution.
Reduced liver function. In mild to moderate hepatic impairment the recommended initial dose for the first 2 weeks of treatment is 5 mg / day. Depending on the individual patient response, dosage may be increased to 10 mg / day. In severe hepatic impairment caution should be exercised in the titration.
Reduced activity of the isozyme CYP2C19. For patients with low activity of isoenzyme CYP2C19 the recommended starting dose for the first 2 weeks of treatment is 5 mg / day. Depending on the individual patient response, dosage may be increased to 10 mg / day.
Discontinuation of treatment. Upon termination of the treatment the dose should be gradually reduced over 1-2 weeks to avoid a withdrawal syndrome.
